Community Teams to facilitate discharges or support post discharge in Gloucestershire
Rapid Response Urgent Care Service
An urgent care service, working 24 hours a day, 7 days a week specialising in management of acute illnesses in adults within the community, preventing unnecessary admission to acute hospitals. Referrals are triaged and prioritised.
They have access to:
- non-medical prescribers
- near patient testing, diagnostic and blood analysis
- the provision of home oxygen therapy and nebulisation treatment.
They work with:
- Frailty Assessment Service
- South Western Ambulance Service Trust
- palliative care teams
- Heart Failure, Diabetes and Community Respiratory and District Nursing teams
- out of hours and patient’s own GP.
Criteria for referral
- Referrals can only be made by registered practitioners, for patients presenting with acute illness.
- Patients must be registered with a Gloucestershire GP.

South Cotswold Frailty Service
The South Cotswold Frailty Service is made up of frailty matrons and wellbeing coordinators who specialise in proactive personalised plans for those patients living with frailty and multi-morbidity. The team has prescribers, direct working with the GP responsible for the patient, and they can access EPR and follow their patient's journey working closely with community teams and attending monthly MDTs to improve proactive personalised care planning, and hospital avoidance for those who can be managed in the community.
